By agreeing to foster in partnership with Staten Island Council for Animal Welfare (SICAW), I agree to the following: I understand that this contract is valid for any animal which I foster through SICAW. In the case of fostering, the person(s) agreeing to foster an animal (the foster family) will follow all relevant procedures and guidelines regarding the animal as stated in the SICAW Foster Contract. I understand that fostering for SICAW includes the following responsibilities (other than providing normal care and feeding): As required, take cat(s) to vet for medical exams, fecal testing, giving meds Feed cat adoption center food Contact SICAW's designated photographer for pics and provide a bio so cat can be posted to PetFinder immediately Show at PetSmart and/or Petco at least twice a month Be available to place cat into adoption center in a timely fashion once cage becomes available Show cat(s) in home to prospective adopters Respond to phone calls, text, emails from Clo and/or other Board members Have weekly contact between you and rescuer Keep a log and turn it over for future reference when cat moves on (e.g., medical issues, visits to vet, cat likes/dislikes) Foster (or coordinate with rescuer), to pick up cat for holidays, snow storms, or other emergencies where cat needs to be removed from PetSmart If foster cannot do some of the above, the rescuer will do those activities. All fosters are adopted out through SICAW’s normal adoption process. If the animal cannot be kept by the foster family, the foster family shall notify SICAW immediately. The animal shall not be, abandoned, relinquished to a shelter or to a Rescue group other than SICAW, transferred, or given to any individual. We appreciate your desire to help SICAW by fostering one of our adoptable pets, but in order to ensure the safety of your foster we ask that you review and sign the following terms and conditions. If for any reason you have to return your foster you must return him/her to a SICAW representative. A SICAW representative will be more than happy to take your foster if you cannot keep them but we will require appropriate time to make proper arrangements. SICAW must approve all adoptions; you are not to give your foster to anyone without the approval of a SICAW representative. If you must take your foster to the vet we ask that you take them to an approved SICAW vet. 1. In signing this foster agreement with SICAW, I declare that I am aware that: animals are different from human beings in their responses to human actions; the actions of animals are often unpredictable; children and animals must be closely supervised when interacting; an animal’s behavior may change at any point. I hereby accept possession, legal guardianship and responsibility for the animal I am fostering. On behalf of myself, my heirs, personal representatives and assigns, to the fullest extent permitted by law, I hereby release, discharge, indemnify and hold harmless SICAW and its directors, officers, employees and agents, PetSmart and PetSmart Charities from any and all claims, causes of action and demands of any nature, arising out of, or in connection with, my fostering activities on behalf of SICAW, which activities are known to me, or of which I should know from ordinary experience, training, observation or instruction. 2. All of the information I have provided is true and complete to the best of my knowledge. 3. Should a pet be placed with me it will reside in my home as a pet. I agree to provide the cat(s) with adequate food, water, affection and medical care which will be paid for by SICAW. All fostered animals belong to SICAW, and the rescue may choose to remove them from my home at any time, and for any reason. *
